Your ticket(s) will be mailed to you October 2012 unless specified otherwise in the 'notes' section of the payment form. If you do not receive your tickets, please contact the Jack Webster Foundation office at 604-261-6184 or email@example.com.
Terms and Conditions
This Webster Dinner ticket purchase form is for the use of companies and individuals who wish to attend the annual Jack Webster Awards Dinner. Please specify the number of tickets you would like to purchase and the name and address to which you would like the tickets sent. Please include your email and telephone in order that we can follow-up if questions arise. The Jack Webster Foundation accepts cheques, visa or mastercard in payment of your tickets. You will receive an income tax receipt in January for a portion of the amount paid for dinner ticket(s). If you are donating funds to the Jack Webster Foundation, you will receive an income tax receipt for the full amount immediately after the donation.
You will have up to 5pm six days prior to the event. to cancel your ticket purchase. If your ticket is purchased by Visa or Mastercard, you will receive a credit to your account by November 15, 2012. If your ticket is purchased by cheque, you will receive a reimbursement cheque from the Jack Webster Foundation by November 15, 2012.
